Your First Visit

If possible, please submit your paperwork prior to the day of your appointment as this will streamline your visit. If you have not submitted your registration paperwork in advance, please arrive 30 minutes prior to your appointment time to give adequate time for check-in. If you submitted your paperwork in advance, then arrive 15 minutes prior to your appointment time to allow sufficient time to verify your insurance data, etc.

All New Patients must have their paperwork completed PRIOR to arrival due to limited space and wait time in our lobby. You may be asked to reschedule your appointment if your paperwork is not completed.

Bring With You

A valid photo ID
Your insurance card(s)
A method of payment for your co-pay. Bluegrass Dermatology can accept credit card, cash, or check payment for the co-pay amount, which is due at the time of your visit.
A complete list of your current medications
Registration Forms (unless previously submitted online)
Please double check your insurance card to see if a referral is required by your PCP (your PCP’s name will be printed on the front copy of your card or you may see “referral required”) in order to be seen by a specialist. If so, the referral authorization must be received by our practice PRIOR to your appointment. Insurance plans with this requirement will NOT accept a referral request by our office.
