We ask that all visitors to our office wear a mask. All New Patients must have their paperwork completed PRIOR to arrival due to limited space and wait time in our lobby. You may be asked to reschedule your appointment if your paperwork is not completed. If you have a cough, fever, chills, difficulty breathing, new loss of smell or taste, diarrhea, suspected or confirmed diagnosis of COVID in last 2 weeks, or close contact with someone with these symptoms, please do not come to your appointment – call our office and we would be happy to reschedule your appointment.
